The approval of ultra-high field (7T) magnetic resonance scanners for clinical use by the US Food and Drug Administration (FDA), and the European regulatory authority have opened the doors for more widespread clinical adoption1, 2 particularly with the release of clinical UHF scanners by MR-vendors. Although the majority of clinical MR systems are 1.5, and 3T systems, a number of large hospitals and academic research centers have explored musculoskeletal MR applications at UHF3–9. UHF 7T scanners provide a unique set of advantages over lower field strength scanners, but they also come with a set of technical and safety challenges. The choice of whether to use a UHF scanner is more nuanced and based on the intended application and thus, it is important to understand the advantages, disadvantages and current challenges of 7T systems for clinical use.
