Dermatitis herpetiformis (DH) is an extremely pruritic, papulo-vesicular skin disease characterized by the presence of granular IgA deposits at the dermalepidermal junction (DE J), an associated, most often asymptomatic, gluten-sensitive enteropathy (GSE) and a strong association with the HLA class II antigens HLADR3 and-DQw2 [1, 2]. The presence of IgA, the predominant immunoglobulin of the mucosal immune system, in the skin of patients with DH, coupled with the striking association with GSE has led to the hypothesis that a mucosal immune response plays an important role in the pathogenesis of DH. A role for a mucosal immune response has been further suggested by the observation that not only the gastrointestinal disease, but also the skin disease can be controlled by a gluten-free diet [3, 4]. Since the gastrointestinal disease is asymptomatic in most patients with DH, many choose to be treated with dapsone, which does not alter the gastrointestinal mucosa [3]. Most patients with DH, therefore, continue to ingest wheat protein (gluten), with the resultant ongoing presentation of antigen to an abnormal gastrointestinal mucosa. Patients with DH thus represent a unique model for the study of the gastrointestinal immune response since it relates both to the pathogenesis of DH and other disorders of the mucosal immune system.
