A 60-year-old female presented with a 1-year history of migrating erythema and crusting. She was diagnosed with eczema and treated with antihistamines at a local clinic, with no improvement. Physical examination showed scattered superficial loose blisters, brownish yellow scabs and polycyclic erythema located on her distal extremities (Figure 1a), vulva (Online supplementary Figure S1) and buttocks. The migratory lesions were repeatedly aggravated and relieved and there were erythematous papules around the nasolabial area. Her tongue was scarlet and beef-like in color with a smooth surface (Figure 1b). She also reported bradykinesia, apathy, and depression for three months. She had diabetes mellitus for 1.5 years and used acarbose to control her blood sugar levels; she also lost 20 kg in weight in the past year.
