目的:探讨糖尿病神经源性膀胱(DNB)患者血清同型半胱氨酸(Hcy)、叶酸(FA)、维生素B12(VitB12)的表达变化及其与膀胱功能的关系。方法:选取2018年1月至2019年8月住院治疗的184例2型糖尿病(T2DM)患者为研究对象,其中DNB患者72例(DNB组),非DNB患者112例(非DNB组)。我们对两组患者血清 Hcy、FA、VitB12进行检测,并分析 DNB 患者血清 Hcy、FA、VitB12与膀胱功能的相关性。结果:DNB组患者血清Hcy(22.71±6.84 μmol/L)、空腹血糖(FPG)(8.15±2.07 mmol/L)、甘油三酯(TG)(2.08±0.56 mmol/L)水平显着高于非DNB组(10.06±4.41 μmol/L), 7.52±1.74 mmol/L 和 1.73±0.64 mmol/L) (P<0.05)。 DNB组患者叶酸(FA)(13.62±4.05 μg/L)、VitB12(268.93±107.62 μg/L)和空腹胰岛素(Fins)(0.56±0.24 mU/L)水平显着低于非DNB组(20.11±5.84 μg/L), 358.22±124.68μg/L和0.77±0.32mU/L)(P<0.05)。 DNB组患者膀胱功能性尿道长度(FUL)和最大尿道压力(PMU)均显着高于非DNB组(P<0.05)。 DNB组患者膀胱顺应性(BC)水平明显低于非DNB组患者(P<0.05)。 DNB组患者FA、VitB12水平与BC呈正相关(P<0.05),Hcy水平与BC水平呈负相关,PMU与FA、VitB12呈负相关,PMU与Hcy呈正相关(P<0.05)。结论:DNB组患者Hcy、FA、VitB12表达水平变化较大,且与患者膀胱功能密切相关。
Objective: To investigate changes in the expressions of serum homocysteine (Hcy), folic acid (FA) and vitamin B12 (VitB12) in patients with diabetic neurogenic bladder (DNB), and the relationship with bladder function. .Methods: Chosen for the study were 184 patients with type 2 diabetes mellitus (T2DM) admitted to the hospital for treatment from January 2018 to August 2019, of whom 72 were DNB patients (the DNB group) and 112 were non-DNB patients (the non-DNB group). Serum Hcy, FA and VitB12 were detected in patients of both groups, and the correlation between serum Hcy, FA and VitB12 and bladder function of the DNB patients was analysed in the study. .Results: The levels of serum Hcy (22.71±6.84 μmol/L), fasting plasma glucose (FPG) (8.15±2.07 mmol/L) and triglyceride (TG) (2.08±0.56 mmol/L) in the patients of the DNB group were significantly higher than those of the non-DNB group (10.06±4.41 μmol/L, 7.52±1.74 mmol/L and 1.73±0.64 mmol/L) (P<0.05). The levels of folic acid (FA) (13.62±4.05 μg/L), VitB12 (268.93±107.62 μg/L) and fasting insulin (Fins) (0.56±0.24 mU/L) in the patients of the DNB group were significantly lower than those of the non-DNB group (20.11±5.84 μg/L, 358.22±124.68 μg/L and 0.77±0.32 mU/L) (P<0.05). The bladder functional urethral length (FUL) and maximum urethral pressure (PMU) in the patients of the DNB group were significantly higher than those of the non-DNB group (P<0.05). The level of bladder compliance (BC) for the patients of the DNB group was obviously lower than that of the patients in the non-DNB group (P<0.05). In the patients of the DNB group, the levels of FA and VitB12 were positively correlated with BC (P<0.05), the level of Hcy was negatively correlated with the level of BC, PMU was negatively correlated with FA and VitB12, and PMU was positively correlated with Hcy (P<0.05). .Conclusion: The expression levels of Hcy, FA and VitB12 changed considerably in the patients of the DNB group, and they were closely correlated with patients’ bladder function..
