The widely held but largely intuitive notion that vigorous physical activity occurs more frequently in children and adolescents than in adults is increasingly supported by scientific investigation (1, 2). Moreover, there are intriguing data suggesting that physicalactivity of childhood and adolescence influences the very process of growth and development by modulating anabolic agents at the cellular level (3). Despite the unique and important role of exercise in the lives of children, evaluation of physiologic responses to exercisein children has been almost exclusively based on protocols developed for adults, a population in whom exercise is far less spontaneous and usuaIIy less vigorous. A major premise of this commentary is that most current protocols are inadequate for children and limit the valuable clinical and developmental information that can be gained from in-laboratory exercisetesting. A strategy is presented that utilizes modern technologies and protocols based on developmental and behavioral factors and patterns of exercise actuaIIy encountered in the real lives of children.Perhaps due, in part, to inappropriate testing strategies, clinical exercisetesting in children has not gained widespread acceptance among practicing pediatricians. References to exercisetesting in major pediatric texts are scant. For example, the only indexed use of exercise testing found in a recent edition of Rudolph's Pediatrics (4) is solely focused on evaluation of children with congenital heart disease. But because physical activity is so natural a" task" in the lives of children, exercise testing has the potential of providing important information for gauging the impact of many childhood and adolescent diseases. It is unfortunate that although excellent research has been focused on exercise responses in children with endocrine disorders, obesity, physical disabilities, cancers, chronic lung disease, heart disease, and many other conditions (see reference 5 for review), this knowledge has been slow to reach the practicing pediatrician. Finally, it is becoming increasingly recognized that certain patterns of physical activity in children and adolescents can substantially benefit health later in life (for example, see reference 6); thus, the accurate assessment of physical activity and fitness may become an essential component of preventive health care evaluation.
