项目摘要

吃音症は中学生・高校生の1％程度と多く存在し、小学生時代と比べ、本人の悩みは中高生の時期に一層大きくなる。実際、不登校・中途退学または発言・発表への怖れなどの深刻な悩みに直面しており、支援法の確立は急務である。発表・社交場面において極度の不安・恐怖を感じ、回避してしまう疾患として、近年、社交不安症が注目されている。本研究の目的は、吃音のある中学生・高校生に対する社交不安症を考慮した支援法の開発である。その目的達成のために、１）吃音のある中高生の社交不安症の併存率を算出、２）支援法につながる要因解析、３）主訴と社交不安症の関連を研究し、それらを踏まえて、４）吃音中高生の支援法の開発を行う。以下、研究成果を述べる。１）社交不安症に用いる社交不安尺度LSAS-CA日本語版は、40名の吃音のある中高校生に測定・解析ができた。LSAS-CA日本語版のカットオフ値は先行研究を参考にし、その結果から、吃音のある中高校生の50%に社交不安症の併発があることを発見した。２）社交不安症の有無で話す前、話している最中、話した後の10項目の独自の問診を作成した。その結果、異なる要因として、「吃音が出ると気持ちが落ち込む」「人前で話すと手足の震える」で差があることを発見した。３）主訴として一番深刻な不登校の有無で比較すると、社交不安症の有無が一つの要因であることが発見した。そのため、吃音のある中高生を支援するには、社交不安症の併発を考慮することが必要であることが示唆された。現在、その結果を論文化し、投稿中である。
