项目摘要

近年、我が国では晩婚化、晩産化が進み、高齢不妊が増加している。加齢に伴い、受精卵の着床が低下し、流産のリスクが高まるためであるが、一方で妊娠を成立させる上で性周期を正常に維持することも欠かせない。性周期を発現させる上で体内時計中枢・視交叉上核からのタイミングシグナルが必須であり、更年期に突入すると性周期が不整になるのは加齢に伴い視交叉上核の機能が低下するためである。性周期の正常化・安定化を図るには規則正しく生活することが大切であるが、現代人の不規則な生活やそれに付随する昼夜差の乏しい光環境は視交叉上核を攪乱させてしまうことから、性周期不整や不妊は若い世代まで波及している可能性がある。そこで本研究では体内時計の視点から性周期不整および不妊症の病態基盤を明らかにし、性周期不正の防止策について提案する。まず閉経前の健康な女性をリクルートし、簡便且つ長期間にわたり睡眠覚醒時刻の記録が可能なウェアラブルデバイスを用いて、睡眠覚醒リズム、月経周期、基礎体温リズムなどを1年超に渡って追跡調査を行い、月経周期が乱れる因子について検証を行う。続いて、各被験者の普段の生活リズムに基づき、月経周期の乱れを起こす原因因子を操作することで、月経周期が予想通り、変化するのか否かを検証する。初年度である令和3年度は普段の生活リズムと性周期の関係性について調査を行いながら、次年度以降の研究の効率化を図るうえでの測定、解析方法の改善を行った。
