医療提供や医師研修において、一般外来診療の役割増加に伴い、医師臨床研修制度で「一般外来の診療研修」が2020年よりはじまった。この新たな研修が、如何に行われているかを探索的調査によって明らかにすることが本研究の目的である。合目的に選定した13名の医師を対象にWEB会議サービスを用いて半構造化個別インタビューを行った。対象者は平均年齢51歳（男性、卒後平均23年）、研修医2名、指導医11名:小児科4名、内科3名などであった。インタビューの質問は、一般外来診療研修における、1）方法、2）内容と方法の重要点、3）困難や課題、4）指導医に必須の指導能力などであった。各々約60分を要したインタビューを録音し、データは活字化して内容分析を行った。分析結果のカテゴリー（説明）を順に示す。1）研修方法：指導医の見学、診察、提示、フィードバック（研修医の面接に始まり薬の処方に終わる一連の診察を基本として、症例の難易度や研修医の能力に応じて診療の一部を指導医が行う）。2）重視している内容・方法：重症度把握、臨床推論、意思疎通や対話、指導内容の絞り込み、フィードバック、経験的知見と医学的根拠、患者中心の医療、社会人としての態度。3）課題点：診察を観察できない、研修医の意欲。4）指導医に求められる能力：①教育的態度と技法（成人教育理論や指導方法を用いて、研修医の意欲を引き出しながら外来診療初学者にあった内容を効果的に指導する能力）、②臨床能力（最新の医学的根拠と十分な経験に裏付けられた臨床能力）、③人間性（良好な対人関係を築くことができ、職業意識と自らの医療観を認識して教育に反映させる能力）。一般外来診療の研修は、臨床研修の基本理念や到達目標に沿って実施されていた。目標達成とその際の困難を克服するために、指導医は、教育、臨床、態度の視点から自らの能力をとらえ指導にあたっていた。
