第一研究として、医療施設内の看護職が考える口腔ケアの課題と教育ニーズを明らかにするとともに、看護職への口腔ケア継続教育内容を検討する基礎資料とすることを目的に、200床以上の1,700医療施設内の口腔ケア教育に関わる看護職の責任者を対象に郵送による自記式質問紙調査を行った。調査内容は、責任者が捉える施設内の口腔ケアの実施状況、組織的な取り組み状況、看護職の口腔ケアの知識・技術，教育内容，教育ニーズなどである。結果、有効回答数(率)は421件(24.7％)であった。看護職は、口腔ケアには関心があると認識しているが、十分実施できていない状況にあった。また、責任者が捉えている看護職の口腔ケアに関する知識の理解度については、「誤嚥性肺炎」が最も高く、次いで「口腔ケアの必要性」であった。一方、「口腔ケアが全身に与える影響」や「口腔内疾患が全身に与える影響」に関しては、理解度は半数程度にとどまっていた。口腔ケア技術については、責任者は対象者の嚥下機能や口腔機能を改善する技術に課題があると認識していた。責任者は，根拠をもって口腔ケアを行うために，口腔内疾患が全身に与える影響を理解するとともに，対象のアセスメント能力を向上させたいと認識していた。結論として、組織的に口腔ケアが疾病予防や機能向上に関わる重要なケアであるという認識を高め、実践に生かせる教育の必要性が示唆された。調査結果については、分析途中であり、今後施設規模や歯科・口腔外科の有無との関連について分析を進めるとともに、第二研究である口腔ケアに関する継続教育プログラムの開発に向けて、継続教育ニーズの整理していく予定である。
