项目摘要

高齢社会において、認知症の前段階であるMCI（Mild Cognitive Impairment:軽度認知機能障害）が注目されており、その頻度は15-25%と報告されている。高齢者が多い整形外科領域でもMCIを有する症例の割合は高いと考えられるが、詳細な検討は行われていない。MCIが存在した場合、病状や手術に対する理解が不十分であったり、痛みに対する認知に影響したりして、手術成績不良の原因となる可能性がある。本研究は整形外科で頻度の高い腰部脊柱管狭窄症例を対象に、MCIの有病率を明らかにすること、そしてMCIが手術前後の痛みやQOL、手術成績に与える影響を調査することを目的として研究を開始した。本研究では、MCIの評価として世界的に用いられているMontreal Cognitive Assessment の日本語版(MoCA-J)を用いてMCIの有無を判定する。また認知症の客観的検査として脳脊髄液のバイオマーカー測定を行う。脳脊髄液バイオマーカーと痛みや手術成績との関連については過去に報告はない。脳脊髄液バイオマーカーは正常例でも認知機能の予後予測に有用であり、MoCA-Jと脳脊髄液バイオマーカーを用いることで、正常例の認知機能予後と痛みや手術成績との関連やMCI例での認知症への進行リスクと痛みや手術成績との関連を検討することができる。本年度は、腰椎手術例で症例登録を進め合計43例で手術前のMoCA-Jと脳脊髄液バイオマーカーの測定を行った。現在まで得られた結果をみると腰椎手術例におけるMCIの有病率早く40％と高率であった。MCIと症状との関係を解析した結果、MCIを有する症例では下肢の痺れが有意に強いという結果がえられた。引き続き予定症例数の100例まで症例を蓄積するとともにMCIと術後成績との関連を解析する。

High social and Cognitive impairment (MCI) is reported at a frequency of 15-25%. This year, 43 cases of lumbar spine surgery were registered, and MoCA-J and spinal fluid were measured before surgery. The incidence rate of MCI is 40% and the incidence rate is high.
