项目摘要

本年度の上半期は、地域中核病院から診療所等への情報提供と情報共有に関する調査を継続し、研究成果をまとめた。A県内の5つの医療施設で、地域医療連携に携わる看護師、社会福祉士の計23 名にインタビューを行った。転院先への書類作成にあたり、看護師は、患者の身体状態、吸引回数や褥瘡の処置等のケア方法、および認知障害や精神状態の程度等を記載していた。一方、研究協力を得た施設において社会福祉士が主体となって作成する書類はなく、医師や看護師の作成書類に不足があった場合に、彼らに追記を依頼していた。例えば、介護度、生活環境、転院に対する希望、生活する上での問題等の情報である。機微な個人情報が情報提供書等に記載され転院先に送られているが、その中身は患者や家族に知らされず、また、情報共有の範囲についても、医療機関名は伝えられることがあっても、「誰」に対して情報提供するのか具体的な職種まで伝えていないことが明らかになった。一方、地域医療連携ネットワークの登録は、共有する情報の範囲が示された説明書で、社会福祉士や退院支援看護師が患者や家族に説明し、意思確認が行われていた。これにより、患者や家族は転院先の施設に提供される情報が認識できるため、地域医療連携における情報共有は適切に行われていることが示唆された。下半期は、在宅医療・介護にかかわる医療従事者を対象とし、地域中核病院から提供される患者情報の受け手からみた共有の実態と課題を明らかにするための調査が行えるよう倫理審査手続きを行った。そして、令和5年3月からインタビュー調査を開始し、現在も継続している。その他、研究成果の一部を第42回医療情報学連合大会（北海道・札幌市）においてポスター発表を行い、本研究を進める上で有用な意見を得ることができた。
