【目的】近年、在宅医療の推進に伴い、内服薬の管理だけでなく、栄養管理や疼痛管理のための注射薬の無菌調製・供給など、保険薬剤師の在宅医療への新しい展開が期待されている。しかし、多くの保険薬剤師は、無菌調製の実務経験が浅く、環境設備も整っていないのが現状である。そこで本研究では、地域や経験によらずに一定水準の教育が可能となるように、無菌調製技術の均てん化を目指した無菌調製指導マニュアル、指導用キットの作成など無菌調製技術を指導するための地域医療連携プログラムの開発を目的とした。【方法】福岡市薬剤師会ならびに福岡地区勤務薬剤師会と連携して、これら薬剤師会の会員薬剤師を対象に、無菌調製研修会(講義ならびに実技)を2回開催するとともに、研修会の内容に関するアンケート調査を実施した。次に、研修会で使用した講義スライドを集約し、無菌調製に必要な備品、準備項目(手洗い、調製用ガウン・手袋の装着方法、クリーンベンチ・安全キャビネットの清掃手順など)、高カロリー輸液・抗がん剤調製手順をまとめ、さらに患者へ提供するまでの注意点なども掲載したマニュアル(無菌調製マニュアル)を作成した。さらに、無菌調製研修会用の模擬例題を作成し、模擬例題に応じた指導および実技に用いる薬品・器材のセットキット(指導準備用キット)を作成した。セットキットには、シリンジ操作での薬液の飛散状況や手技の正確性が確認可能な蛍光指示薬入りバイアルを作成し、飛散状況確認のためのUVランプも用意した。【成果】本研究では、無菌調製の一定水準の知識や技術の指導が効率的に実施できるように、無菌調製マニュアルや指導準備用キットなど地域医療連携プログラムを開発した。本プログラムは、その活用により、保険薬局における無菌調製の普及に寄与することが考えられた。

[Objective] In recent years, the promotion of home medicine, the management of internal medicine, the sterile preparation and supply of injectable medicine, and the development of new home medicine for health care professionals are expected. In addition, there are many protective agents, sterile preparation and management systems, and environmental protection equipment. This study aims to develop a regional medical network based on the educational possibility of a certain level of regional medicine, the standardization of aseptic modulation techniques, the guidance of aseptic modulation techniques, and the preparation of guidelines for aseptic modulation techniques. [Method] Fukuoka Municipal Teachers Association and Fukuoka Regional Service Teachers Association conducted a survey on the contents of the seminar, including the opening of two sessions of the seminar and the training of the members of the association. Next, the seminar will be held in the use of lecture materials, intensive preparation, preparation items (hand washing, preparation, bag loading method, cleaning procedures for safe preparation), high-quality infusion, anti-drug preparation procedures, and patient care. In addition, the simulation examples for aseptic modulation training are prepared, and the simulation examples are prepared for guidance and technical preparation of products and equipment. The UV light source is used to confirm the dispersion status of the liquid and the legality of the operation. [Results] This study aims to develop a certain level of knowledge and technical guidance for aseptic preparation, as well as the implementation of effective aseptic preparation. This article discusses the application and protection of aseptic preparation.
