项目摘要

わが国の認知症施策では認知症の早期診断・早期対応が推奨されているが、認知症の診断を受けてから介護サービス等を利用するまでの診断後支援が未整備である。そのなかで、外来看護師は認知症の診断直後から継続的に認知症の人と家族に支援できる立場にあり、認知症診断後支援の役割を担うことが期待される。そこで本研究の目的は、①認知症の人と家族が認知症の診断を受けてから介護サービス等を利用するまでの期間に求める診断後支援を明らかにすること。②認知症外来に勤務する看護師による認知症診断後支援モデルを作成してその実用性を検討することである。前年度に分析した研究目的①の認知症の人の介護家族8名を対象とした調査の結果、多くの対象者は認知症の診断を受けた直後には専門職から的確な支援を得られず、独力で手探りのまま情報収集をしていた。そのなかで家族の思いに寄り添い一緒に対応を考えてくれる専門職に信頼感を抱いていた。一方で、認知症外来受診の際に看護師から支援を受けたと感じている対象者は少ない現状も明らかとなった。そこで令和4年度は、研究目的②にむけて、認知症外来で診断後支援を実践した経験のある専門性の高い看護師6名を対象としたインタビューも実施した。SCATによる分析の結果、全対象者の理論記述は認知症診断後支援を実践するためのシステムづくりでは6項目、認知症の人と家族への支援内容については13項目に分類された。そこでは認知症診断後支援では、診断後の早い時期に認知症の人と家族を相談に繋げる必要があるため、対象となる人を見逃さず、認知症診断直後から相談しやすい関係を構築することが重要な鍵となることが明らかとなった。今後は、２つの調査結果をもとに認知症診断後モデル案を作成し、その実用性について検討していく。
