本研究の目的は,認知症高齢者が落ち着いて過ごすことができる状態が一定して変わらずに維持される暮らし,すなわち「定常化された暮らし」の要素をとらえ,ショートステイを利用する認知症高齢者を安定した暮らしへ導くための援助について検討することであった。対象者は認知症高齢者とその家族の2事例(A氏,B氏)であった。研究方法は,対象者がショートステイを利用する前・後の自宅での面接およびショートステイ利用中の観察データにもとづき,施設入所中の様子と自宅での様子が異なる場面を事例的に分析した。認知症高齢者が安定した暮らしを営むために定常化することが望ましい要素として以下のようなものが見出された。A氏は日常生活の殆どに援助を必要としており,「時間のズレによる排泄や睡眠の変調」「食事内容や介助方法の違いによる食事拒否」が見られた。B氏は自宅での日常生活動作の殆どは自立しており,「自由に行動でき自立性が保たれると落ち着く」,「さりげなくサポートしてくれる存在により安心する」という様子がみられた。また,2事例に共通して,「普段のデイケアで利用している場所にくると安心できる」という様子がみられた。これらの結果より,日常生活に多くの援助を必要とする場合には生活時間や介護方法を一定に保つことが重要であると考えられる。よって,自宅での生活時間や介護方法についての詳細な確認を行い出来るだけ再現することが必要である。また自宅である程度自立している場合には,その自立性を保つことが重要であり,誇りを傷つけないように分かりやすい表示やさりげない誘導などが必要である。そして,ショートステイにおいて定常化された暮らしを維持するためには,家族と施設職員との情報交換の重要性が示唆された。
