项目摘要

高齢者ショートステイ施設は、近年の在宅医療の推進や在院日数の短縮、変化する家族機能や家族構成などにより、自宅での介護だけでは負担の多い方を対象に短期間施設に入所して、介護を受けることができる施設サービスである。本施設は、家族の介護負担の軽減から始まったサービス提供施設ではあるが、今後は自宅での介護が継続できるための支援を行うサービスとして位置づくことが必要であり、そのためには、スタッフの抱える入所時の困難点である急変時対応の体制づくりが急務である。本研究の目的は、高齢者ショートステイ施設に勤務する看護職・介護職が円滑に連携し、早期かつ適切に急変時対応を実施するために必要な急変時対応スキルを明らかにし、教育プログラムを検討することである。今回Covid-19が流行したことにより、主にCOVID-19感染対策の指導のため訪問巡回し、高齢者ショートステイ施設の感染対策の実施状況および看護職・介護職の連携・協働の様子、看護師等スタッフの精神的疲労・負担状況等の現状把握を実施し、今後の教育システム構築に向けた有用な情報を得ることができた。同時に、高齢者ショートステイ施設は、今まで自宅にいた方の出入りとなるため感染リスクが高く、他施設と比しても早々に一時閉鎖されてしまう施設となっていた。また、他施設の人員確保のためスタッフの移動等を余儀なくされていた。高齢者ショートステイ施設の特徴を考えた急変時対応のスキルアップへの影響要因が明らかになった。それらを踏まて、現在は急変時対応のスキルの内容・教育方法の再検討を文献や専門家、スタッフの意見をいただきながら実施しているところである。
