本研究は高齢血液透析患者の主体的自己管理行動を支援するヘルスコミュニケーションモデルの開発を目指している。今年度は第１段階の高齢血液透析患者の療養生活におけるヘルスリテラシーの構成要素を明らかにすることを目的とした。そこで、日本透析医学会が公表している教育関連施設にて治療を受けている65歳以上の高齢血液透析患者16名を対象にインタビュー調査を実施した。研究参加者はいずれも適切な自己管理行動を実行しながら療養生活を活き活きと過ごされていると施設管理者が推薦する方々であった。インタビュー内容は、療養生活で（意識して）行っている自己管理行動についての「情報の入手、理解、評価、活用」とした。インタビューの内容を分析した結果、家族や友人、地域活動への参加、人との交流、治療を受ける以前の趣味や楽しみのある生活を継続すること等が生きる支えや生きがいとなっており、自己管理行動の継続や改善を促進していることが明らかとなった。また、透析導入前から今に至る療養生活の失敗や困難の経験から学びを得て、新たな課題を明確にして対処しており、その都度課題を乗り越えながら今の自分があることが語られていた。そこには患者自身だけでなく医療者や家族、友人のサポートによる自己管理に関する内容の吟味、何をどのようにしていくかといった意思決定、工夫しながら実行している自己管理行動の様相があるということが詳らかとなった。今後本研究で明らかとなった内容を学会で報告する予定としている。

The patient himself or herself is responsible for the content, taste, and time of the patient's management. In the future, the content of this study will be determined by the Society.
