全身性野生型トランスサイレチンアミロイドーシスは年齢と共に有病率は増加し、80歳以上でその有病率は約10-25%と推定されているが、日常診療における診断率とは大きなギャップがある。これは、アミロイドーシスの確定診断には生検が必要不可欠であり、高齢者が多いこの疾患では消化管粘膜生検や心内膜心筋生検の施行はリスクが高く、施行が困難であることが影響していると思われる。そのため、非侵襲的イメージングが注目されており、近年99mTcシンチグラフィーはトランスサイレチンアミロイドーシスの診断に有用であることが報告されている。また、アミロイドーシスは予後不良であることが知られているが、トランスサイレチン心アミロイドーシスに対するタファミジス治療は、プラセボ群と比較して30ヶ月死亡を30%低下、心血管関連入院を32%低下させたことが報告されており、早期診断・早期治療が望まれる。しかしながら、タファミジス治療を行っても治療効果の指標はなく、治療反応例の臨床背景についても明らかになっていない。我々は、99mTcシンチグラフィーの心臓集積活性が早期診断や重症度に関連すると考え、心臓集積の評価法、心臓集積に関連する因子、生命予後との関連を検討する。まず、2006年から2021年までの15年間にアミロイドーシスが疑われた肥大心症例のううち、99mTcシンチグラフィーを施行された約200症例を抽出した。病理学的にトランスサイレチン心アミロイドーシスと診断された症例、ALアミロイドーシス症例、非アミロイドーシス例の臨床データについて調査し、トランスサイレチン心アミロイドーシスの臨床的特徴について検討を行なっている。同時に、タファミジス治療を導入した症例を対象に経時的に臨床データの評価・検討を行っている。
