项目摘要

免疫疫抑制剤による易感染状態や拒絶反応のリスクを持つ腎移植患者は、QOLの維持向上を図るため適切な自己管理行動が必要となる。しかし、自己管理行動がQOL（健康関連QOL）と生体データ（腎機能、BMIなど）、医療費（出来高医療費）にどう影響しているか明らかにした研究はない。本研究の目的は、腎移植患者における自己管理行動がQOLと生体データと医療費に及ぼす影響を３年間の追跡調査によって明らかにすることである。本研究の対象者は、2018年6月時点で腎移植外来に通院する腎移植患者全数であり、そのうち研究協力の得られた132名を分析対象とする。データ収集方法は記名式の自記式質問紙調査と電子カルテデータ・レセプトデータの収集である。健康関連QOLの測定と生体データの収集は質問紙調査と電子カルテから、2018年6月、2019年6月、2020年6月の３地点で入手することが出来た。レセプトデータの収集は、申請者が医事運営課保険担当より2018年6月～2021年6月まで入手する事が出来た。収集したデータ項目は、対象者の基本属性（性別、年齢、身長、体重、ドナーソース、急性拒絶反応経験、移植前透析期間、移植後経過期間、合併症の有無）、健康関連QOL、生体データ（BMI、血圧、血糖値、中性脂肪、腎機能）、医療費として、医科点数表による出来高情報（EF統合ファイル）である。調整変数を投入した混合効果モデルの結果、生体データと健康関連QOLに自己管理行動と時期の交互作用を有意に認め、医療費は傾向が確認された。適切な自己管理行動が腎移植患者の精神的・社会的QOLの維持向上に関連した。また同じく適切な自己管理行動が外来医療費の削減に関連することが示唆された。この結果は、腎移植患者の適切な自己管理行動を支援する看護活動の基盤となるエビデンスになり得ると考える。

Immunosuppressive agents are necessary to prevent infection and to maintain QOL in kidney transplant patients. QOL (health-related QOL), biological health (kidney function, BMI), medical expenses (high medical expenses), etc. The purpose of this study was to investigate the effects of QOL, health care costs, and self-management on kidney transplant patients over a 3-year period. The subjects of this study were all kidney transplant patients admitted to the hospital by June 2018, and 132 subjects were analyzed by the joint study. Data Collection Method: A Self-Recording Question Paper Survey and an Electronic Data Collection Method Health related QOL measurement and biological data collection paper survey were conducted in June 2018, June 2019 and June 2020. From June 2018 to June 2021, the applicant will be responsible for the medical operation department. Collection of data items, basic attributes of subjects (gender, age, length, weight, gender, acute rejection, dialysis before transplantation, post-transplantation, presence or absence of complications), health-related QOL, biological parameters (BMI, blood pressure, blood glucose, neutral fat, renal function), medical expenses, medical points table, high information (EF integration). Adjustment of inputs, mixed results, biological data, health-related QOL, self-management actions, and time interactions Appropriate management actions are associated with maintaining the mental and social QOL of kidney transplant patients. The same management actions are associated with the reduction of external medical expenses. The results showed that the nursing activities of kidney transplant patients were supported by appropriate self-management actions.
