项目摘要

本年度は、前年度に行なった東北メディカル・メガバンク計画三世代コホート調査参加者のうち、妊娠高血圧症候群（HDP）患者約二千名を対象にした、有害アウトカムに関連する特徴を使用したクラスタリングで得たクラスタを詳細に解析したところ、臨床的多様性を説明可能なクラスタが得られなかった。そのため、臨床的多様性に対応する層別化を行うため、以下の解析を実施した。HDP患者の妊娠40週までのすべての妊娠週数の病態が、既存のどの病型分類の定義に当てはまるかを、アメリカ産婦人科学会の病型分類の基準に従い同定した。これにより、HDP患者の妊娠中の病態の経時変化を反映した時系列フェノタイプを得た。得られた、HDP患者の時系列フェノタイプに対して階層的クラスタリングを実施し、得られた階層および病態変化のパターンから、8種類の時系列サブタイプ（temporal-subtype）を見出した。得られたサブタイプの臨床的特徴を解析したところ、最終的に判定される病型が同じでも、病態が変化した妊娠週数、病態が変化してから出産までの時間、HDPの重要なアウトカムの一つである早産や低出生体重の割合が、大きく異なることを示した。病態変化の時期が遅く、病態変化から出産までの期間が短く、低出生体重の割合が多い時系列サブタイプをsevere temporal-subtype、そうでない時系列サブタイプをmild temporal-subtypeとし、得られた8つの時系列サブタイプは、HDPの臨床的多様性を反映していることを示した。また、時系列サブタイプのより詳細な特徴の理解のために、関連する妊娠中の曝露因子を探索し、時系列サブタイプに特徴的な、妊娠早期のいくつかの血液検査項目や生活習慣などの曝露因子を見出した。
