がん治療後の循環器疾患患者の心不全発症・増悪因子の自己管理支援プログラムの策定にむけ、疾患・治療内容、症状、服薬、心理的項目、身体活動性等の身体的項目、社会的活動や社会的サポート状況等の社会的項目など、がん治療後の特性を踏まえた身体面・心理面・社会面での内容を明らかにするため、化学療法を実施した経験のある乳がん患者200名を対象とした質問紙調査を実施した。その結果、化学療法中と化学療法後に感じたことがある症状は、いずれも「倦怠感」が最も多く、次いで「身体活動の低下」「むくみ」であり、化学療法中、化学療法後ともに、ステージが上がるにつれて、何かしらの症状を感じる割合が高くなる傾向があった。教育機会については、化学療法をうける際に、心臓への影響についての説明を「医療者から受けた」割合は4割程度であった。化学療法前・化学療法中、化学療法後に医療者からの教育機会の有無については、化学療法前・化学療法中では約4割、化学療法後では5割程度であり、およそ半数程度の人は教育機会を有していないと回答した。教育内容では、化学療法前・化学療法中は、「治療に伴う症状の観察」が約3割で最も多く、次いで「バランスのよい食事」、「日々の身体チェック」であり、「心不全症状の観察」は1割程度であった 。化学療法中や化学療法を受けた後に、むくみや倦怠感の症状を感じる患者は多いが、治療に伴い心不全症状やその観察についての教育を受けている人は少ない状況であることが示された。これらのことから、医療者、患者ともにがん治療後のリスクへの知識やセルフモニタリングの教育機会の強化するプログラムの必要性が明らかとなった。この結果をもとにプログラム内容の充実にむけて検討を実施する。
