リアルワールドデータである診療報酬請求データベースを活用し国内におけるパブリックヘルスの喫緊の課題を迅速に評価検証する研究を行う予定であったが、申請者の退職による応募資格喪失によって1年間での研究廃止となった。以下、これまでの研究によって得られた結果について報告する。近年、日本で異性間接触による梅毒患者が増加しているが、日本では世界で標準的に行われている性感染症の伝播を遮断する公衆衛生対策である、患者のパートナーへ感染リスクを通知する仕組みがなく、パートナーに対する医療的ケアがどの程度なされているか明らかではなかった。そこで本研究では加入者台帳のある民間診療報酬請求データベースを用い、新たに梅毒治療を開始した患者の配偶者の梅毒検査実施状況を評価した。診療報酬請求データベースから2010年から2017年の間に新たに梅毒治療を開始した患者とその配偶者を同定、配偶者の梅毒検査は治療開始後3か月以内の非トレポネーマまたはトレポネーマ試験のいずれかの実施と定義した。217人の新たに治療を開始した患者のうち、29人(13.3%)の配偶者が梅毒検査を受け、うち男性患者-女性配偶者のペアでは12.6%(23/182)、女性患者-男性配偶者のペアでは17.1%(6/35)が梅毒検査を受けた。44歳以下の女性配偶者のサブグループでは、近年の梅毒報告症例の増加と対策強化にもかかわらず配偶者検査の割合が経年的に低下した。梅毒患者のパートナーへの適切なケアを確保するために、日本でもパートナー通知サービスの実装を検討すべきと考えられた。
