项目摘要

がん診療連携拠点病院（以下、拠点病院）制度は、拠点病院を中心とした患者の集約化と、がん医療の均てん化を目指している。これまで、大阪府下でがん診療を行っている全ての病院を対象に治療件数や拠点病院区分と生存率の関連性を分析した。次に、拠点病院のみを対象とし、各病院で行われた全てのがんに対する手術件数と3年生存率の関連性をもとに最低満たすべき手術件数を推定した。今年度は、同じ手法を用いて、大阪で罹患数の多いがん（胃、大腸、肺、乳房）とハイリスクな治療を要するがん（食道、膵臓）の手術件数と5年生存率の関連性をもとに、拠点病院が最低満たすべき部位別年間手術件数を推定した。その結果、最低必要な手術件数は食道がん35-39件、胃がん20-25件、大腸がん25-29件、膵臓がん10-14件、肺がん10-14件、乳がん25-29件であると推定された。望ましい生存率を得るために拠点病院が年間に実施すべき手術件数はがんの部位によって異なっていた。また、国内外ですでに運用されているがんの部位別最低手術件数と類似しているものもあった。
