本研究課題は、特定臨床研究として長崎大学臨床研究審査委員会の承認および研究機関の長の許可を得た。特定臨床研究として申請するにあたり、交付申請書に記載した研究実施計画を以下のように改変する必要があった。具体的には、研究対象施設を長崎大学病院に加えて諫早総合病院歯科と佐世保市総合医療センター歯科の3施設とし、各実施医療機関機関で全身麻酔下で歯科治療、口腔外科処置を行う12 歳以上60 歳以下の自閉症スペクトラム症の患者で、不眠症の診断でロゼレム以外の薬剤で内服治療を行っている患者、および過去に睡眠障害と診断された既往は無いが、入院前にアテネ不眠尺度で4点～5点の「不眠症の疑いあり」あるいは6点以上の「不眠症の可能性が高い」患者を対象とすることとした。研究同意を得た患者に対し、メラトニン受容体アゴニスト（ロゼレム）を持参薬に上乗せして追加投与する実験群に割り付けられた患者は、全身麻酔7日前から術後7日後までロゼレム1回8mgを14日間、就寝前(20:00時)に持参薬の不眠治療薬と一緒に経口投与する。ロゼレムを不眠症治療薬に追加投与しない対象群に割り付けられた患者は、持参薬の不眠症治療薬のみを全身麻酔7日前から術後7日後まで連続して14日間、処方どおり経口投与することとした。各群45例でサンプルサイズを設定し、全体で90例を設定した。以上の研究計画を2020年9月に論文タイトルStudy on the preventive effect of ramelteon on the onset of sleep disorder after general anesthesia in patients with autism spectrum disorderとして英文雑誌Medicineに誌面報告した。現在、対象患者の睡眠データの収集および解析を進めている。
