项目摘要

我々の研究チームはこれまで、国際共同研究を組み、面接時間を短縮できる社交不安症向けの次世代遠隔型の認知療法プログラムを開発した。本研究では、開発したプログラムが日本人の社交不安症およびひきこもり状態にある重度の患者に有効であるか検討することを主な目的とする。昨年度に引き続き、今年度は外来診療で対応可能なレベルにある社交不安症を対象に、次世代遠隔型認知療法プログラムの有用性・安全性・忍容性を検討するパイロット試験（前後比較デザイン）を実施した。本試験は、6都県に所在する9つの医療施設で実施している。一連の遠隔プログラムを終了した被験者に対しては、「プログラムを受けたことでの生活上の変化」や「プログラムに継続的に取り組む上で手助けになったこと／困難だったこと」「プログラムの全般的な評価」などについて、電話調査（30分）を実施した。長期化する新型コロナウイルス感染症の拡大に伴い、国内での被験者のリクルートが想定よりも遅れているものの、目標症例数（12～16例）の9割程度に達した。次年度には全例のリクルートならびに介入が完遂する予定である。パイロット試験の結果を踏まえ、ランダム化比較試験の詳細をつめていく。また、引きこもり状態にある社交不安症への適応を検討する上で、まずはひきこもりの当事者支援の専門家が、次世代遠隔型認知療法プログラムを外来診療で対応可能なレベルにある社交不安症患者1名への介入を開始した。
