项目摘要

医療的ケア児の家族の語りとして、2023年1月までかけて医療的ケア児を育てている親42名の声を集めた。この語りを文字起こしし、トピックごとに分けて分析し、わかりやすい形で分類し、さらに専門家による修正も行った。その過程で、国会等の立法措置の問題、行政の政策課題、各自治体の担当者等の運用、解釈の問題などについて検討した。研究期間中には、2020年6月に医療的ケア児支援法が成立するなど、社会的には注目される動きも起こったが、それが実際の個別の家族が実感できる形となっているかといえば、そうではないことも理解された。その背景として医療的ケア児といっても、障害のレベルはもちろん、肢体不自由の場合、知的障害を伴う場合、肢体や知的の障害はなく、たんの吸引などの医療的ケアのという場合もある。インタビューでは、個別性の高い事例でかつ、社会的には少数者であることで、支援の制度が不足していることに加え、担当者ごとに理解にばらつきがあることが指摘された。また、教育現場における安全重視の傾向から送迎バスに乗れない問題や、通学手段として介護タクシーへの補助が出ている自治体が出てきているものの、その介護タクシーの事業者や付添の訪問看護師の確保は個々の親に任されており、実態としてまったく利用できていないという問題も理解できた。2021年6月に医療的ケア児支援法が成立するとの立法措置がなされ、予算も確保できているのに、医療的ケア児をとりまく状況はそれほど変わっていない。制度と資源に関する情報提供をセットにしていく必要や、行政担当者による理解のばらつきなどについても指摘された。情報整備や、支援者教育の必要なども次の課題である。
