项目摘要

本研究は、自宅以外の在宅として多様化している高齢者向け住宅の特に食事や栄養支援に着目した研究である。高齢者向け住宅は、地域包括ケアシステムの中心に位置付けられている。地域包括ケアシステムでは、高齢者向け住宅に暮らす高齢者が、介護保険施設や医療施設への入退院時にも継続したサービス提供が可能となるよう、施設間の情報共有を図る必要がある。そこで、本年度は本研究に先駆けて実施した介護保険施設と医療施設を対象とした食品軟化剤の使用に関する質問紙調査の内容を分析し、施設側で提供されている食事内容について検討した。さらに、食品軟化剤を使用して食事提供を行っている介護保険施設の協力を得て、入居者を対象とした舌圧測定を実施し摂食嚥下機能評価を行った。食品軟化剤は、調査対象とした施設の約半数が使用しており、施設区分と施設規模別で有意差はなかった。給食の運営形態別では、軟化剤使用ありは委託で多く、直営で少なかった。食品軟化剤の主な使用食材は、肉類であり、その他に魚や根菜類であった。食品軟化剤を使用していない理由は、食品軟化剤を使用していない施設のうち半数の施設が「高価である」と回答した。次いで、「調理工程を工夫して対応できている」と「作業が煩雑になる」との回答であった。食品軟化剤は、対象施設の約半数で使用されていたが、価格や作業面に関する課題が示された。
