尿失禁の症状軽減には、行動療法による治療が効果的な介入として推奨されている。本研究では、症状軽減に寄与するとされる推奨行動を、外来患者が日常生活の中で実践していくことを支援するため、行動科学に基づく簡便な支援法を開発し、それを泌尿器科における通常の外来治療に加えることによるセルフケア行動の促進効果と心身の症状改善効果を検討することを目的としている。2022年度は、尿失禁症状を主訴とする外来患者を対象として、本研究で開発した外来診療で簡便に実施できる行動療法によるセルフケア実践支援法を用いたランダム化比較試験（RCT）を実施し、その介入効果を検討するためのデータ収集を行った。具体的には、2020年度に準備・作成した本研究調査用の質問紙バッテリーやセルフケアの実施支援ツールを用いてランダム化比較試験（RCT）による介入効果の検討を実践した。研究計画に基づき、本大学病院泌尿器科外来を受診中の女性尿失禁患者の中で、骨盤底筋訓練や習慣行動の改善などにより症状の軽減が期待できると主治医が判断した患者のうち、研究参加同意の得られた者に対し、1年にわたる縦断的な質問紙調査と体組成測定を行った。また、RED-Capシステムを用いて、研究参加者を無作為に介入群または非介入群（対象群）に１：１で割り付け、介入群には行動療法によるセルフケア実践支援のための個別面談を２回加えた。面談では、質問紙調査と体組成測定の結果に基づき、日常生活の中で実践可能な訓練や習慣行動の具体的な目標設定と行動実施のためのアドバイスを行い、セルフモニタリングによる行動実施支援を提供した。現在は、両群における習慣行動やモチベーション、体組成や心身症状の経時的変化を観察し、統計分析による比較検討を実施するためのデータを集積中である。
