项目摘要

本研究は、慢性閉塞性肺疾患（以下COPD）を抱える高齢患者に対する在宅呼吸リハビリテーション（以下呼吸リハ）を推進するため、在宅呼吸リハビリの利用実態を明らかにすること、さらに、在宅呼吸リハの根幹となる在宅版セルフマネジメント教育プログラムを開発することを目的とした。令和2年度は、医療レセプト・介護レセプト連結データを用いた研究を行い、在宅呼吸リハを提供する主要な医療介護サービスである訪問リハ及び訪問看護利用者の特性を縦断的に調査した。その結果、訪問リハ・看護を利用するCOPD患者は死亡率が高く、重症者が多い可能性が示唆された。続いて、在宅呼吸リハを提供する理学療法士・作業療法士・看護師に、セルフマネジメント教育を含む在宅呼吸リハの実践内容と効果・課題についてインタビュー調査を行った。その結果、在宅呼吸リハの実践では、“息苦しさに対処しながら「なんとか生活していく方法」を一緒に見出”していた。理学療法士・作業療法士・看護師は、在宅呼吸リハによる利用者の行動面での変化や精神面での変化を認識していた一方で、より効果的な介入を行うためには、重症な利用者に対するケアの標準化や疾患管理に関する医師との協働体制には課題があると考えていた。二つの研究結果から、在宅で行うCOPD患者に対するセルフマネジメント教育プログラムの発展のため、重症な利用者の暮らしを支えるプログラムの構成として、利用者のありたい姿を捉えた上で、心身の土台作りをするケアや、生活する中で自然と呼吸リハビリテーションが行われるような環境調整を取り入れていく必要があると考えられる。さらに、薬物療法や急性増悪時の対応等、疾患管理に関する医師との協働を推進する内容についても検討が必要である。

The purpose of this study is to promote the development of in-home respiratory status in patients with chronic pulmonary disease (COPD) and to develop in-home respiratory status education. In 2002, the Ministry of Health conducted research on the characteristics of medical care users, and conducted research on the characteristics of medical care users. Results, visits, and care utilization indicate that COPD patients have a high mortality rate and a high likelihood of severe disease. In addition, the Department of Health provides medical therapists, occupational therapists, nurses, and health care professionals with a comprehensive range of training programs, including in-home nursing practice, results, and research. As a result, the practice of breathing in the house is reversed, and the "method of living" is gradually emerging.
