出産体験が原因となるPTSDは産後の母子や家族のwell beingに悪影響を与える公衆衛生上の重要な問題である。これまでに行われた研究より、産科合併症、新生児合併症、緊急帝王切開などが出産後のPTSDのリスク因子となっていることがわかった。しかし、状況の変化が激しい周産期の現場であらかじめ合併症や分娩様式を予測することには限界がある。また、正常分娩においても、産後PTSDを発症したり、症状を呈したりするケースが報告されていることから、分娩様式や合併症などによる対象者の制限を行わないRCTの必要性が示唆された。介入の筆記表現法はPennebakerの提唱した方法と産後の集団を対象とした先行研究を参考に産後入院中の実施を含む1回以上、1回15～30分程度の実施とした。産後入院中にリクルート、ベースラインの評価、介入を行い、産後1か月・3か月時にフォローアップを実施した。リクルートした345名のうち、回答率は1か月フォローアップ時点で91%（313名）、3か月時フォローアップ時点で82％（282名）であった。解析の結果、プライマリアウトカム（トラウマ症状）に対する介入・非介入群の有意差は1か月・３か月ともに認めなかった。また、セカンダリアウトカム（産後鬱症状）の結果も同様に有意差を認めなかった。出産体験のとらえ方は文化や価値観によって大きく異なる。本研究は産後PTSD予防に向けた国内で初めての介入研究であり、欧米諸国で行われている先行研究との結果の違いにも注目し解析を進めることで産後母子保健のエビデンス確立に貢献することが期待される。また、本研究では産後PTSDのローリスク群を対象としているが、緊急帝王切開後の褥婦などのハイリスク群での研究の必要性も示唆された。

The causes of PTSD and the impact of postpartum maternal and family well-being are important public health issues. This study focuses on obstetric complications, neonatal complications, emergency surgery, and postnatal PTSD factors. The condition of the child is changed, and the birth date is changed. The necessity of RCT is demonstrated by the restriction of the target for normal delivery, postnatal PTSD symptoms, presentation of symptoms, and presentation of complications.
