项目摘要

近年、Evidence based Medicine (EBM)実践の重要性が高まっているものの、歯科は個人診療が主であることから大規模臨床研究が行われにくく、一般歯科診療の多くは限定的エビデンスに基づく。医科では、医療ビッグデータを用いた大規模臨床研究が盛んであり、治療効果、危険因子、予後予測、費用対効果等について多数のエビデンスが創出されている。一方、歯科の詳細情報を含む大規模データベースは世界にも存在しない。全国を網羅する歯科データベースの構築により、大規模な歯科臨床研究の飛躍的増加と歯科EBMの進展が見込まれているものの、その有用性や保持すべき情報は明らかでない。本研究の目的は、大規模歯科データベース構築の臨床研究への有益性を定量的に評価検討するとともに、その保持すべき規模や項目を明らかにすることである。現在までに病院データを用いた、歯科病名、歯科診療行為の妥当性についての検討、歯科レセプトデータを用いた疫学研究の実施可能性について検討を行い、学会発表、論文公表を行ってきた。2022年度は公的データを用いて前年度のレセプトデータを用いた研究の再現性について検討を行った。

Medical doctors and doctors use large-scale models to study clinical symptoms, treatment results, risk factors, future outcomes, and so on. On the one hand, the information of the department of science and technology requires that there are significant changes in the size of the world. In the national network, the development of EBM and the usefulness of the national network and large-scale research departments are very important to ensure that the information is clear. In this study, the purpose of this study is to study the effectiveness of the large-scale model department in the study of the effectiveness of the quantitative analysis and maintenance of the standard specification project. At present, the use of medical records, the names of medical diseases, the names of medical diseases in general hospitals, and the use of epidemiological epidemiology in medical hospitals are now available for the study of the possibility of clinical practice, medical records, and documents. In the year 2022, the government will use the previous year to conduct a study on the nature of the disease.
