高齢者の口腔機能低下は誤嚥性肺炎や低栄養を引き起こすことから、適切な歯科医療の供給が推進されている。そのような状況の中で、これまで国内外で歯科介入が全身状態改善に与える効果が研究されてきた。一方、嚥下障害や心血管疾患等の基礎疾患を有する高齢者における一般歯科診療の安全性については十分な議論がなされていない。後出血、術後感染、異物誤嚥をはじめ、歯科診療に伴う有害事象や偶発症は多くの小規模研究で報告されているものの、大規模な調査はほとんど存在しない。歯科は個人診療が主であり大規模臨床研究が困難であることに加え、診療に起因する有害事象・偶発症は回避すべき対象であるため、調査時の過小報告も懸念される。本研究の目的は、大規模歯科レセプトデータベースを用いて、基礎疾患を有する高齢者に対する歯科診療の実態、歯科診療に関連する有害事象や偶発症の発生率、およびその関連因子を明らかにし、高齢者歯科診療の安全性を向上することである。2022年度は大規模レセプトデータベースを使用して、抗凝固薬使用者における抜歯後出血の検討、体重減少とその後の要介護の関連について検討し、学会発表、論文公表を行った。

The oral function of patients with hypertension is low, and the supply of appropriate dental care is promoted. The results of dental intervention at home and abroad are studied. The safety of general dental treatment is highly discussed in the case of underlying diseases such as prescription, swallowing disorders and cardiovascular diseases. Post-operative bleeding, postoperative infection, foreign body ingestion, adverse events associated with dental treatment, and occasional complications are reported in small and large studies. Dentistry has difficulties in conducting large-scale clinical research on individual diagnosis and treatment, causes of harmful events, occasional diseases, avoidance of adverse events, and small reports during investigation. The purpose of this study is to clarify the status of dental care, the incidence of adverse events and complications associated with dental care, and the associated factors in the use of large-scale dental care, the presence of underlying diseases, and the improvement of the safety of dental care. In 2022, we will conduct research on the relationship between large-scale use of anti-coagulation agents, post-dental bleeding, weight loss, and post-dental care, academic development, and paper publication.
