项目摘要

「食形態」に影響を及ぼす「歯の状態」について、「歯の状態」と要介護高齢者の移動能力および認知機能の自立性とは関係があると仮説を立て、特別養護老人ホーム入所者1,000名を対象に調査した。「食形態」とは、「常食」または「特別」を示し、自歯または義歯の適合に焦点をあてて調査した。義歯適合良好者は、屋内歩行移動の自立が21.6%と最も高く、意思伝達66.2%、日課理解47.5%、年齢をいう36.2%、直前理解45.0%、自分の名前をいう85.7%、今の季節の理解43.7%、場所の理解46.4%と、他の「歯の状態」と比べて「できる」割合が有意に高い結果であった。本研究により、「歯の状態」と移動能力および認知機能との関係が明らかとなった。自歯ではなく、義歯適合良好が心身機能の自立性が高い結果について、自歯での残存歯数が影響していると考えられる。正常な歯を有する人の咀嚼する能率を100%とすると、1本欠損した人では約半分に低下し、複数欠損した場合には約7割程度も低下するとされている。また、咬合力は残存歯数と正の相関関係を示し、残存歯数が多いほど咬合力が強いといわれている。義歯適合良好は、欠損のある自歯より咀嚼力や咬合力が高いことが、今回調査した移動能力や認知機能の自立性の高さと関連していると考えられる。さらに、義歯適合良好者は義歯調整により咀嚼が活発化し、常食摂取により良好な栄養状態が保たれていることも示唆される。義歯適合良好は、咀嚼の活発化や咬合支持による良好な栄養状態と関係すると考えられる。要介護化リスクのフレイル予防のためにも栄養状態を良好に保つ必要がある。「歯の状態」を良好に保つことにより、要介護化を防ぐことが示唆された。
