高齢者の要介護化リスクであるフレイルは、生活機能障害、要介護状態、死亡などの転帰に陥りやすい状態とされ、地域在住高齢者における頻度は7～10%、75歳以上の高齢者における頻度は20～30%で年齢とともに増加する。しかし、生理的な老化と区別して早期に発見し、栄養と運動の介入により要介護化を防止することも可能であるといわれている。要介護化リスクのフレイル診断指標は標準化された基準がなく、Cardiovascular Health StudyによるCHS index、Study of Osteoporotic Fractures によるSOF index、自己記入式基本チェックリスト、簡易版フレイルインデックス等が開発されている。本研究では2022年度は、介護予防の二次予防のみでなく、一次予防（一般介護予防）対象者をスクリーニングできる生活評価の示唆を得るため、健康高齢者を対象としてインタビュー調査を実施した。健康高齢者の語りから、高齢者自身が加齢に伴い自覚する生活の変化と心身機能低下について明らかにし、要介護化リスクファクターの示唆を得ることを目的とした。静岡県三島市在住の健康高齢者20名、東京都文京区在住の健康高齢者20名、合計40名で、65歳以上75歳未満の男女各10名ずつ、75歳以上85歳未満の男女各10名ずつとした。対象者40名に、インタビューガイドを用いて60分間程度の個別インタビューを実施した。調査内容は、厚生労働省により要支援者をスクリーニングする標準指標として示されている基本チェックリストおよび基本属性の聞き取りと、半構造化面接として5年前と比べて加齢に伴い自覚する外出や友人との交流、運動機能、栄養や口腔機能、物忘れ、生活の充実感や疲労感について自由に語っていただいた。今後は、基本チェックリストの集計とICレコーダーに録音したインタビュー内容について分析する。
