项目摘要

当該年度は、令和3年度実施したパイロットスタディ“在宅重症児の学童期のきょうだい支援オンラインプログラム”をベースに倫理委員会への申請、承認を受け、臨床試験登録システム登録後にプログラムを実施した。プログラムの目的は、楽しみながら仲間がいることを知り、きょうだい自身のもつ悩みや喜びを共有することである。対象は、18歳までの重症児がいる8～12歳のきょうだいとその親である。楽しみながらきょうだいが交流を図れるように構成し、3か月間、毎月１回、計3回の参加を基本とした。評価は、きょうだいと親に、参加前後でKINDLR小学生版QOL尺度、交流会各回終了時に自作の評価アンケート、プログラム終了後にきょうだいと親それぞれにインタビューを実施した。プログラムは、2022年9月～11月に開催した。きょうだいの参加人数は計6名（小学3年1名、4年3名、5年1名、女児3名、男児3名）であった。全員が重症児の姉・兄の立場であった。各回参加人数は2～4名、重症児の医療的ケアは呼吸器や経管栄養などであった。プログラム参加前後のQOL尺度の総得点平均は、きょうだい77.3点／76.5点、親74.４点／74.5点で、参加前後で差はなかった。きょうだい用アンケートでは、「参加することができてよかった」が満点であった。親用アンケートの自由記載では「初対面でも緊張せずに仲良しに」、「オンラインでも楽しく参加」、「重症児のことを話せてうれしそう」、「家族だから話せないことを話したり聞く機会に」、「親がきょうだいの思いを知る機会にもなり安心に」が抽出された。オンライン開催により、きょうだいの思いや考えを親も知る機会になったことが示唆された。プログラム終了後のインタビューにはきょうだいと親それぞれ3名が参加した。引き続きインタビューの分析を進め、プログラム評価をおこなう。
