【目的】本研究は、在宅重症児の被虐待に対する訪問看護師（以下訪看）の気づきから確信までのプロセスを「被虐待気づきの指標（被虐待38場面）」を用いて、被虐待に気づき始めた場面は何か、それを確信につなげるための意図的な観察場面は何かを経時的に明らかにする。【意義】在宅重症児の被虐待場面を38に集約し、その場面を訪看が把握したか否かを確認できる指標を開発した。指標は、在宅重症児の訪問実績のある全国の訪看を対象とした調査で、その妥当性と信頼性を得た。本研究では、訪看を対象として、被虐待の気づきから確信までのプロセスで把握した被虐待場面を具体的に示す。訪看が被虐待のグレーゾーンへの気づきを確信していく様を共通の指標で明らかにすることは学術的にも施策的にもその意義は大きく被虐待の早期発見の客観的な指標となり、在宅で療養する者の被虐待対策支援に貢献できると考える。【2021年度の成果】調査対象の訪問看護ステーションを選定し、プレテストを実施。【2022年度の成果】小児の訪問を実施している関東の訪問看護ステーションを選定し、「被虐待気づきの指標（被虐待38場面）」を用いて在宅重症児の被虐待に対する訪看の気づきから確信までのプロセスを調査した。10名からの訪問看護師から3件の事例に対する3－6カ月間の調査結果を得た。訪問看護師の概要は、看護師歴平均16.3年、訪問看護師歴平均8.1年、重症心身障がい児の訪問平均8.9件、被虐待児訪問平均1.7件であった。重症心身障がい児の被虐待場面を認識した時期は、88.2％が訪問開始後であった。38の被虐待場面は1場面でも認識をしていれば被虐待を疑っていたが確信につながっていた場面は、「障がいや発達に合わせたコミュニケーション場面を見つけてもらえない」、「オムツ、洋服を着替えがされていない」「清潔の保持がされていない」であった。
