项目摘要

本研究の目的は、生活習慣病予防の保健指導である特定保健指導を取り上げ、保健指導参加者の家族への波及効果も意図した保健指導技法を開発することである。家族を「波及を受ける人」とし、「波及を受ける人」に着目した保健指導の波及プロセスを解明し、そこから保健指導技法を抽出し、保健指導技法の効果を検証する。具体的な研究手順は、①保健指導の波及プロセスの解明、②保健指導参加者・家族に対する保健指導の波及の実態調査、③保健指導技法の開発、④保健指導技法の効果検証である。令和4年度は、③④を実施した。特定保健指導の従事者（保健師、管理栄養士）を対象に「家族への波及を意識した保健指導」をテーマに、特定保健指導における家族の波及の現状や課題についてグループミーティングを2回行った。1回目は6名、2回目は7名の参加であった。その結果、「家族への波及を意識した保健指導」として、「保健指導参加者から家族への働きかけを増やすこと」「家族を巻き込むような行動目標を設定すること」「保健指導に参加していることを家族に意識づけること」にまとめられた。この保健指導技法の効果を検証するために、特定保健指導の従事者がこれを実践し、特定保健指導に参加することに意思を示した者（保健指導参加者）の初回面接時期（1回目）と3～6か月後の評価時期（2回目）の計2回、保健指導参加者と配偶者に対して質問紙調査を行う。調査内容は、保健指導参加者には、生年月日、性別、体重、支援レベル（動機付け支援・積極的支援）、日本語版健康増進ライフスタイルプロフィール（JLV-HPLP Ⅱ）、配偶者には、生年月日、性別、体重、特定保健指導の参加経験、治療状況、日本語版健康増進ライフスタイルプロフィール（JLV-HPLP Ⅱ）等である。現在は、1回目の調査を実施している。
