项目摘要

本研究の目的は、認知機能が低下した軽度認知障害高齢者の食行動異常に対して、摂食嚥下機能評価、認知機能評価、嗅覚評価、食行動評価、脳血流評価を用いて評価し、軽度認知障害高齢者の摂食行動障害のメカニズムを明らかにし、予防的介入への道筋を明らかにすることと、安全な食事摂取環境のあり方を検討することである。本年度は、昨年に引き続き、予備調査で作成した調査票を用いて、研究協力施設のリハビリテーション病院ならびに老人保健施設に入所ならびに通所中の基礎疾患にアルツハイマー病、前頭側頭変性症、レビー小体病、脳血管障害の既往をもつ軽度認知症患者のうち改訂長谷川式簡易知能評価スケールにおいて15/30点以上、CDR0.5~1.0の教示の理解が可能な軽度認知機能障害高齢者に対し、本調査を実施していたが、昨年来のコロナ感染症の蔓延により調査予定施設において通所中患者のデーター収集が予定通り行えなくなったため、昨年度、研究期間の延長を申請し、これまでに得られた資料を引き続き解析した結果、軽度認知障害高齢者の摂食行動障害は、認知機能検査、意欲検査と摂食嚥下機能検査から解析が可能であることがわかった。このため、対象者数を増やすため、主な研究協力施設である福岡県内のリハビリテーション病院に入院中または、広島県内の介護老人保健施設に入所している認知機能障害をもつ摂食嚥下障害高齢者に対してデーター収集し、認知機能や食行動を含めた日常生活動作の違いによる嚥下機能の改善を嚥下造影検査によって測定することとした。しかし、再び感染力の強いオミクロン株の流行によって、介護老人保健施設でのデーター収集の中断を繰り返している。この軽度認知症患者を対象とした検討で、認知機能、意欲、食行動の特性と嚥下機能の改善について分析を加えこれらのデーターから、軽度認知障害高齢者の摂食行動障害のメカニズムを明らかにしてゆく予定である。
