项目摘要

本研究課題は特定機能病院(SFH: Special Functioning Hospitals)精神科部門における精神科看護実践の向上を目標として、同科看護実践向上教育プログラムの開発を目的としている。2018年度は第一段階であり、日本全国のSFH精神科部門の病床規模などの施設特性、診療対象とする患者・疾患・治療内容等の概要と、実際の看護実践の内容等について明らかにした。翌2019年度は第2段階であり、1）複数のSFH精神科部門看護管理者を対象として、看護実践上の困難やニーズ等の聞き取り調査を行い、この結果を踏まえ、2）ある一SFHの精神科部門中堅看護師を対象として高難度精神科看護実践能力等について聞き取り調査を行い、発達パスおよび関連促進・阻害要因等を明らかにした。2020年度は本研究課題の最終段階として、上記1）および2）の結果を踏まえて、全国の全SFH精神科部門看護師を対象として、SFH精神科部門における実践内容・能力獲得・向上ならびに関連要因等に関する大規模調査を行い、教育プログラム案の精錬を予定していた。しかし、2020年度ならびに2021年度はCOVID-19蔓延の影響を受けた。2022年度は、影響は軽減されたものの、本研究課題の対象であるSFHは各地域における高度感染対策や人命救助の高度医療提供の核であること、そして、研究者らのヒアリングでは精神科病棟の入院数制限、外部との連絡制限によりグループホームへの見学や自宅への外出訓練等の退院に向けた支援や連携等の看護実践の減少等が認められた。このような状況から、SFHに対する調査については2022年度も倫理的配慮ならびに調査の妥当性の観点から取りやめ翌年度へと延期した。2022年度はこれまでの調査結果の発表準備に取り組み、SFH精神科部門に関する既存資料のレビューは学術雑誌にて論文として成果発表を行った。

This study aims at the development of psychiatric nursing practice and peer nursing practice in psychiatric departments of Special Functioning Hospitals (SFH). In 2018, the first phase of SFH Psychiatric Department in Japan was completed, including the summary of hospital bed size, facility characteristics, treatment objectives, patients, diseases, and treatment contents, and the contents of actual nursing practice. In the second stage of the next fiscal year, 1) a number of SFH psychiatric department nursing managers have reported on difficulties in nursing practice, etc., and 2) a number of SFH psychiatric department core nurses have reported on difficulties in nursing practice, etc., and have reported on the investigation of difficulties in nursing practice, etc. In 2020, the final stage of this research project, the results of 1) and 2) above, the implementation of large-scale investigation and the refinement of educational programs in all SFH psychiatric departments nationwide, the implementation of content, ability acquisition, upward and related factors in SFH psychiatric departments, etc. The impact of COVID-19 spread in 2020 and 2021 was also felt. 2022 - 2023: The impact of this study is decreasing, and the targets of this study are: SFH, high infection control, high level of medical care provision, and the number of admissions to psychiatric hospitals. The external communication system is limited to the following aspects: visits to schools, home training, etc., retirement, support, connection, etc., reduction of nursing practice, etc. The status of the investigation, SFH investigation and evaluation of ethics in 2022, the appropriateness of the investigation and evaluation of the next year's delay. In 2022, the preparation of the report of the investigation results was carried out by the SFH Psychiatric Department, and the existing data were submitted to the Academic Journal.
