本研究は、臨床研究と基礎研究、双方から、感染性胃腸炎に対する五苓散注腸の効果のエビデンスの構築をすることを目的として行うものである。近年、感染性腸炎などに対する経口補液療法が浸透し、点滴に頼らない治療が患児や保護者にもたらす恩恵を実感する一方、依然点滴が必要な症例も少なからずいる。五苓散の注腸投与が小児の感染性胃腸炎に有効であるという報告をもとに施行したところ、点滴の適応症例がさらに減るように感じた。しかし、この治療法は一般的ではなく一部の医療施設や医師のみで行われている状況である。五苓散による治療の普及には、エビデンスの不足も障壁となっており、エビデンスの構築が必要である。まず、医師主導臨床研究を計画し、生後半年から5歳以下の感染性胃腸炎の患児を対象、プラセボ実薬対照ランダム化盲検試験を行っている。昨年度は、コロナの流行により小児の感染症が激減し、胃腸炎も同様に少なかったため、症例登録が得られなかった。基礎研究においては、既にロタウイルスでの研究実績がある、大阪大学微生物病研究所のウイルス免疫分野の小林剛准教授の研究室に協力を仰いだ。ロタウイルス（SV11）による胃腸炎のマウスモデルの作成やウイルスの操作方法について連携し、愛媛大学でのロタウイルスによる胃腸炎のマウスモデル作成に成功した。このモデルに五苓散を投与した。下痢の評価として、濾紙上に採取した1回分の便をスライドガラスを載せて圧排し、便と水分の面積を測定した。しかし、下痢の改善は認められなかった。罹患率の高い小児胃腸炎に対する治療法の改善は、小児にはもちろんのこと、小児を支える家庭の負担を減らし、経済効果、少子化への抑制への手助けにもつながり、現在の日本社会において有益な研究と考えられる。今後も引き続き臨床研究の症例登録と基礎研究を継続する。
