项目摘要

静脈内鎮静法は歯科においても治療を円滑かつ安全に遂行するために広く用いられているが、静脈麻酔薬による呼吸抑制に加え、特に歯科領域では歯牙切削時に口腔内に注水を行うため誤嚥等のリスクが高く、慎重な呼吸管理が必要とされる。しかし、一般的に用いられている経皮的酸素飽和度測定は無呼吸低呼吸などの呼吸異常を迅速かつ正確にとらえることは出来ない。研究代表者は先行研究で簡易睡眠検査装置の鼻フローセンサーを用いた呼吸モニタリングにより、経皮的酸素飽和度測定だけでは発見できない呼吸異常を発見できることを示した。本研究の目的は鼻フローセンサーのモニタリングにより、呼吸異常の変化を早期発見し対処することで、鎮静中の呼吸異常の頻度を減らせるかをランダム化比較試験で検証する。また呼吸異常の頻度と関連する背景因子の検討も行い、安全な静脈内鎮静法のための新たな呼吸モニタリングとリスクスクリーニングの手法を確立することである。本研究期間中に発生したCOVID19の感染拡大の影響を受け、被験者のリクルートに大きな障害が起こった。これを踏まえ、2020年度中には研究計画を一部修正しサンプルサイズの縮小、外部睡眠検査機関の活用等の変更を行い、研究倫理審査委員会の承認を得た。2021年度は昭和大学江東豊洲病院での研究実施体制の整備を行った。被験者のリクルートとデータ取得は2022年度から開始したが、口腔外科担当医異動の影響によりリクルートが進まなかった。2023年度も引き続きリクルートとデータ取得を進め、解析を行う予定である。現在の進行状況を踏まえ科研費による研究計画を1年延長した。
