患者がいだく「他者に世話になることの負担感（Self-perceived Burden：SPB）」の増減を主評価とし、援助的コミュニケーション研修の効果の検証を目的とした。研究デザインはロールプレイを用いた実験的デザイン、対象者はエンドオブライフケア研修（以下ELC研修）を受講する看護師40名、臨床経験年数10年以上を参加要件とした。患者役として模擬患者（SP）を起用し、家族に介護負担をかけはじめているがん患者を演じてもらい、看護師とオンラインを用いた面接を実施し映像として記録した。ELC研修を受講前の看護師、受講後の看護師のいずれと面接を行うかについてSPには知らせなかった。SPは、本研究用に作成したシナリオを基に、専門のコーディネーター、トレーナーの元で対応や演技のトレーニングを行った1名とした。解析にはSPBスコアを目的変数にした二項ロジスティック回帰分析を行った。またSPがロールプレイ中に感じた看護師のコミュニケーション特徴（反復、沈黙、視線など7項目）とSPB（高・低）の関係性を解析する共起ネットワーク分析を行った。実施したオンライン面接は38（対受講前看護師17、対受講後看護師21）回であった。ELC研修受講前・後の看護師年齢（44.3vs46.7）、臨床経験年数(20.0vs20.5)、直近3か月間の看取り患者数（3vs2）で、統計的な差はなかった。ロジスティック回帰分析の結果、SPBスコアに対し、ELC研修受講前・後、緩和ケアに関する知識高・低、積極的傾聴態度尺度；ALAS高・低のいずれも有意な影響は認められなかったが、受講後看護師との面接でSPBが高くなる傾向が見られた。共起ネットワーク分析の結果、受講後看護師との面接におけるSPB高と関連するのは、看護師が行う反復、沈黙、一方SPB低と関連するのは視線、反復、傾聴姿勢、沈黙であった。
