近年、骨盤痛における病態の悪化や痛みの憎悪に対し、痛みを伝達する中枢神経系の変調(中枢性感作)の関与が指摘されている。この疾患概念では、局所の痛みの改善のみを治療対象とするのではなく、ADLやQOLの改善等、心理社会的問題を包括的に捉えたアプローチが重要となっている。そこで本研究課題では、骨盤痛の心理社会的側面に焦点を当てた生活影響度を把握する新たな評価尺度「Pelvic Pain Impact Question(PPIQ)」日本語版の妥当性・信頼性の検討を行い、加えて骨盤痛に対する中枢性感作を明らかにすることを目的とした。その研究実施計画は以下のとおりであった。【1)骨盤痛による生活影響度を評価するPPIQ日本語版の妥当性及び信頼性の検討】インターネット上に専用サイトを作成し、オンライン上でアンケートを実施することとした。専用サイトへの誘導は、Facebook・Twitter等のSNSを通じて拡散し、骨盤痛者に対して一般情報(年齢、性別、骨盤痛の診断名)及びPPIQ日本語版を評価する予定であった。【2)骨盤痛による生活影響度と中枢性感作との関連性の検討】PPIQ・CSI・DASS-21(不安、うつ、ストレス)を実施し、さらに中枢性感作の生理学的指標として用いられる定量的感覚検査を評価することにしていた。圧痛閾値(Pressure Pain Threshold：PPT)は、上腕骨外側上顆より遠位(長橈側手根伸筋筋腹)に疼痛が10段階中2となる圧力を測定し、また、時間的加重(Temporal summation：TS)として、PPTの強度で10回繰り返し刺激1回目と10回目の疼痛強度の変化率を算出予定であった。しかし、定量的感覚検査を評価する目的で圧力痛覚計を購入したが、研究代表者の病気療養のため、本研究課題の実施計画については未遂行のうちに中止するに至った。
