PTTMはがんに随伴し、担がん患者の予後に大きな影響を及ぼす可能性のある致死性疾患である。診断、治療法も確立されておらず、早急な対策が望まれる。本研究ではPTTM症例を前向きに登録し病態解明、診断法確立と治療戦略開発を行い、診療現場への啓発と予後改善を目指す。病態解明の一環として各種バイオマーカー測定や病理組織の検討、治療としてこれまでPTTMに奏功したとの報告のあるイマチニブ投与を行い、有効性および安全性を検討する。初年度は研究体制の整備および全国の基幹病院への説明会を行い、当院へPTTM2例紹介、登録となった。2022年度は引き続き症例蓄積を行い、新たに4例が紹介、登録となり、各種バイオマーカーの測定、治療としてイマチニブの投与が行われ、生存期間を含む主要および副次評価項目を観察した。現在も1例の観察が進行中である。詳細な解析は症例集積後に行う予定となっているため、研究成果がまとまってから報告予定である。また、学会シンポジウムで本研究について発表し、症例蓄積を呼びかけた。本研究によりPTTMのイマチニブの有効性が確認されれば、治療法がないPTTMの予後改善に寄与することができる。病態解明されればより効果的な治療薬選択につながる可能性がある。蓄積されたPTTM症例を解析することで診断基準の作成につなげることも考えている。症例の蓄積が課題となるため、引き続きPTTMの啓発、関連診療科や医療機関との連携強化を行い、症例登録を目指す
