项目摘要

戦後日本の生活保護の捕捉率は、先進諸外国より低いものの、「医療ニードには積極的に対応してきた」といわれている。実際、路上で暮らす生活困窮者の保護開始場所として医療機関が多くなっていたり、生活保護の受給が決定されにくい稼働年齢層の男性でも、医療ニードがあれば受給が決定されやすくなっている。医療扶助単給の実績も生活扶助以外の他の扶助に比べて多くなっており、医療扶助費は生活保護費のなかで最大費目となっている。ではなぜ戦後日本では、医療ニードには積極的に対応する生活困窮者処遇が展開されることになったのか。本研究の目的は、医療ばかり頼らない支援体制の構築に向けて、上記の問いに答えを提示することである。そこで本研究は、「なぜ戦後日本では医療ニードには積極的に対応する生活困窮者処遇が展開されることになったのか」という問いを解明するため、「戦後日本において医療扶助は誰にどのように給付され、誰をどう処遇してきたのか」という作業課題を、2022年度から2024年度までの間に、段階的に検討することにした。2022年度は、作業課題のうち「医療扶助は誰にどのように給付されてきたのか」を検討した年度であった。まず、戦前から戦後にかけて、生活困窮者処遇において政策的な医療給付がどのように展開されたと説明されているのかを先行研究をもとに整理し、レビュー論文として公表した。次に、生活保護行政に長年携わった経験のある行政職員にインタビュー調査を実施し、行政職員の立場から「医療扶助は誰にどのように給付されてきたのか」という問いに関する意見を聞き取った。加えて、医療扶助受給経験のある当事者にインタビュー調査をするために支援団体にコンタクトをとり、関係性の構築を図った。以上が2022年度の実績の概要である。
