项目摘要

601名のせん妄患者を対象に、妄想が顕在化するせん妄のサブタイプについて妄想の有病率、臨床的特徴、危険因子の検討を行った。Delirium Rating Scale-Revised-98を用いて妄想を評価し、重回帰分析を使用して個々の要因とせん妄患者における妄想の関連を検討した。また、妄想形式の特徴を調査した。その結果、78名（13.0％）のせん妄患者に妄想がみられ、高齢（p<0.001）、女性（p<0.001）、一人暮らし（p<0.001）がせん妄患者における妄想の発現と有意に関連していた。妄想形式は、迫害や被毒等の関係妄想に分類され，84.3％の患者が迫害者は医療スタッフであると確信していた。妄想形式や危険因子から，遅発性パラフレニーや接触欠損パラノイドの特徴との類似性が示唆され、せん妄患者の妄想への対応や治療においては、妄想の背後にある孤立感や不安、恐怖を考慮した心理的介入が必要であると考えられた（Tachibana M. et al. Int J Geriatr Psychiatry. 2022 Jun;37(6)）。せん妄の類型化の検討に関連して、急性脳機能障害であるカタトニア（医学的疾患あるいは精神疾患による）とせん妄の類似性と鑑別、現行の診断分類（DSM-5）の問題について整理を行った。現行の診断基準におけるカタトニアとせん妄は実臨床ではしばしば重複し、せん妄の存在はカタトニアにおける医学的疾患の存在を強く示唆する指標となると考えられた（Tachibana M, et al. Front Psychiatry. 2022 Apr 7.)。また、せん妄が、各疾患における死亡率、施設入所、および認知機能低下に与える影響に関するレビューを作成した（Tachibana M. et al.Psychogeriatrics. 2023 Jan;23(1))。

The prevalence, clinical characteristics, and risk factors of delusions among 601 patients were investigated.
