VHL病は、生涯にわたり全身に腫瘍を多発する遺伝性希少難病であり、約半数で網膜血管芽腫を合併し、失明に至っている症例も少なくない。日本での網膜血管芽腫の疫学は不明であり、体系的な研究はされておらず、その全容は明らかにされていない。本研究の目的は、VHL病における眼病変の評価系の確立することで、その病態を解明し、早期発見の手段、治療の適応判断に発展させることである。全世代のVHL病患者における網膜血管芽腫について、未発症の段階も含めた経過、治療効果を複数の眼科検査モダリティで評価し、他臓器病変との関連性、血族患者間の関連性を含めて解析する。本年度は57人のVHL病患者を対象として、視機能の評価、網膜血管腫の有無を診断した。網膜血管腫を認めた37人(64.9%)60眼において、複数の眼科検査モダリティ（眼底写真、蛍光眼底造影検査、光干渉断層計、網膜光干渉断層計血管造影(OCTA)）を用いて、網膜血管腫を評価した。VHL病の網膜血管腫の臨床像として、従来VHL病で特徴的とされてきた丸く隆起した境界明瞭な血管腫nodular typeとは異なる、平坦な異常血管増殖を示すflat typeがあることが明らかとなった。flat typeはOCTAを活用することで明瞭に描出された。

The purpose of this study is to make sure that the symptoms of VHL disease and ophthalmopathy are confirmed, explained, early, treated and judged. In the whole generation of patients with VHL disease, the vascular germs of the whole generation of patients with VHL's disease were diagnosed, the undiagnosed segments were complicated, the results of treatment were complicated, the relationship between patients with other diseases, and the relationship between kinship patients were analyzed. In this year, 57 patients with VHL disease had abnormal symptoms, such as abnormal symptoms, mechanical abnormalities, and abnormal vascular diseases. There were 37 patients (64.9%) with vascular recognition and 60 eyes with complex ophthalmology (fundus photography, optical fundus angiography, optical dry cut-off angiography (OCTA)). In patients with VHL disease, the blood vessels in the network of patients with VHL disease are like the disease, the enlarged blood vessels of the patients with VHL disease are clearly defined, and the normal blood vessels of the patients with flat type disease are flattened to show that the blood vessels are normal. Flat type "OCTA" use the picture clearly to draw the picture.
