プロトンポンプ阻害薬（以下PPIとする）は、胃酸分泌を抑制することにより、幅広い胃酸関連疾患の予防および治療において重要なキードラッグであり、その処方は日本を含めた全世界で増加し続けている。PPIの過剰処方・不適切処方は世界各国で指摘されているが、日本におけるPPIの過剰処方・不適切処方については検討されていない。これは、日本では保険病名がつけられ、PPIの不適切処方が表に出にくいことによると思われる。そこで本研究では、①日本のPPI処方は過剰か、全国の一般市民を対象としたPPI処方の実態調査の結果を国際比較により検討し、②日本のPPI処方は保険診療として適切か、単施設のレセプトデータを用いた調査により検討し、③なぜ不適切処方が起きるのか、患者ならびに医師を対象としたインターネットを用いた全国調査で明らかにする計画である。PPIの不適切処方の実態と背景を明らかにし、それを抑制する方策を提言することにより、医療適正化を図り、現在の日本の医療に急務である国民医療費削減に貢献することを目的にしている。今年度は目的②を明らかにするため、島根大学医学部附属病院におけるPPI処方の実態調査を行うための準備を進めた。具体的には、島根大学医学部の医の倫理委員会に本研究の実施に対する申請を行い、承認を得た。なお、本研究は、PPI処方とレセプト病名および投与期間やフォローアップの内視鏡検査が適切に行われているか、等を検討する内容であるため、当院で行われている医療の適正評価の側面を持ちうり、このため、審査にやや時間を要した。しかし、保険病名と実際に医学的に妥当な医療プラクティスが必ずしも一致しているとは限らず、また、医学的に適切なプラクティスであっても保険診療としては不適切な場合もあることから、本研究の結果が、一概に提供されている医療の適正評価には必ずなるとはいえないと考える。

The hospital's evaluation of medical appropriateness was based on patient name and duration, and the examination time was important. The results of this study provide a comprehensive assessment of the medical appropriateness of the treatment.
