医療経済評価によるエビデンスに基づいた適切な医療資源の分配を目指し、放射線診断学領域、特にIVR領域での医療経済評価を実施した。研究実施計画で設定した下記3項目の研究1）原発性アルドステロン症診療における選択的副腎静脈サンプリングと経皮的焼灼術、2）腎細胞癌に対する凍結療法術、3）慢性血栓塞栓性肺高血圧症に対する血管拡張術、のうち2022年度は、原発性アルドステロン症診療における選択的副腎静脈サンプリングと経皮的焼灼術の費用対効果分析を主に実施した。研究方法としては、東北大学病院にて2020年から2023年の間に実施された原発性アルドステロン症に対する副腎静脈サンプリング、及び経皮的副腎焼灼術のデータを後ろ向きに収集した。収集したデータから、選択的副腎静脈サンプリング及び経皮的副腎焼灼術を含めた診療フローと、既存の診療フローでの費用対効果を比較した。解析の結果、選択的副腎静脈サンプリング及び副腎焼灼術を含めた診療フローは、既存の診療フローと比較し、Incremental cost effectiveness ratio (ICER)が男性1,113,391 JPY-per-QALY、女性924,756 JPY-per-QALYという結果だった。これはsocietal willingness-to-pay thresholdとして設定された5,000,000 JPY-per-QALYを下回る結果となった。上記研究結果から、選択的副腎静脈サンプリング及び副腎焼灼術を含めた原発性アルドステロン症診療フローは、既存診療フローよりも費用対効果に優れる診療フローであることが明らかとなった。

Medical evaluation in the field of radiology and IVR is carried out based on the allocation of appropriate medical resources. The study implementation plan is set as follows: 3 studies: 1) primary renal failure diagnosis and treatment of selected accessory renal vein and cauterization of skin; 2) freezing therapy for renal cell carcinoma; 3) vascular dilation for chronic thromboembolic pulmonary hypertension; and 2022. The cost-effectiveness analysis of primary renal failure diagnosis and treatment was carried out. Methods: From 2020 to 2023, Tohoku University Hospital conducted a series of primary and secondary renal vein cauterization procedures. To compare the cost of accessory renal cautery, including diagnosis and treatment, with that of existing diagnosis and treatment. The results of analysis, selection of accessory renal veins, and accessory renal cautery were compared with those of existing treatments. The Incremental cost effectiveness ratio (ICER) was 1,113,391 JPY-per-QALY for men and 924,756 JPY-per-QALY for women. The societal willingness-to-pay threshold is set to 5,000,000 JPY-per-QALY.
