本研究は「循環機能不全終末期患者の褥瘡発生リスク因子を探索すること」を目的としている。日本ではBraden Scaleを褥瘡リスクアセスメントスケールとして使用している病院が多いが、栄養状態が低下し悪液質が出現した終末期患者に対して、Braden Scaleだけでは褥瘡発生の予測が難しいと考えている。悪液質が出現しやすいという終末期特有の状況や浮腫が出現しやすいという循環機能不全特有の状況を踏まえ、特殊な機械や検査に頼らない方法であるADLや血液データなどを使用して褥瘡発生要因を探索する計画である。この課題は後方視的カルテ調査である「循環機能不全終末期患者の褥瘡発生予測因子を探索する研究」および「浮腫を伴っている循環機能不全増悪期患者の褥瘡発生予測因子を探索する研究」、前方視的観察調査である「浮腫を伴っている循環機能不全増悪期患者の体圧を測定する研究」で構成している。2022年度は文献を用いて研究計画の再検討を行うとともに、後方視的カルテ調査の研究体制の構築および倫理申請を主に実施した。日々治療薬が開発される中で比較群をどのように設定するのか、多寡がないようにどのような調査項目を挙げるのか、について文献を使用して再検討した。再検討した結果「循環機能不全終末期患者の褥瘡発生予測因子を探索する研究」および「浮腫を伴っている循環機能不全増悪期患者の褥瘡発生予測因子を探索する研究」の調査期間や調査項目を決定した。その後、研究実施機関（病院）と相談し、倫理申請に必要な研究実施機関所属者に研究協力を依頼し、研究体制を構築した。研究体制構築後に所属学部への倫理申請を行い、承認後に研究実施機関（病院）へ研究協力を依頼し、研究実施の許可を得た。

The purpose of this study was to explore factors contributing to decubitus ulcer development in patients with terminal circulatory insufficiency. In Japan, the Braden Scale is used to predict the occurrence of bedsores in many hospitals and in patients with advanced stages. The condition of fluid quality appearing in the terminal stage, edema appearing in the terminal stage, circulatory insufficiency, special mechanical examination, method, ADL, blood pressure, use, etc. This project consists of a retrospective-view survey entitled "Study on factors predictive of decubitus genesis in patients with terminal circulatory insufficiency", a study on factors predictive of decubitus genesis in patients with edema associated with circulatory insufficiency at the advanced stage, and a forward-view survey entitled "Study on body pressure measurement in patients with edema associated with circulatory insufficiency at the advanced stage." In 2022, the re-examination of the research plan for the use of literature and the construction of the research system for the review of literature and ethics were carried out. Daily treatment is developed in the middle of the comparison group, the number of settings, the number of investigation items, the use of literature, and re-examination. The results of the re-examination were: "Study on the exploration of pressure ulcer development prediction factors in patients with terminal circulatory insufficiency" and "Study on the exploration of pressure ulcer development prediction factors in patients with edema and circulatory insufficiency in advanced stage." Research institutions (hospitals) should discuss with each other, research institutions should cooperate with each other, and research institutions should be established. After the establishment of the research system, the ethics application of the affiliated department is carried out, and after the recognition, the research organization (hospital) is supported, and the permission for research implementation is obtained.
