成人期アトピー性皮膚炎症例の中にステロイド外用剤を用いた治療に抵抗を示す症例が多く存在するが、この抵抗の程度は症例によってまちまちである。またそれら難治例の多くはその臨床経過中に多種のステロイド外用剤を長期間にわたり使用している場合が多い。他科領域からはこれまでにグルココルチコイド受容体の個体間差異やステロイド剤によるグルココルチコイド受容体のdown regulationについて多くの報告がなされているが本症の成人難治例についてはそれらが検討された報告はなかった。そこで、ステロイド外用剤を用いた治療に抵抗を示す成人期アトピー性皮膚炎症例におけるグルココルチコイド受容体の量的なあるいは質的な変化を調べることを今回の研究の目的とした。昨年度は基礎研究として、正常成人末梢血単核球を用いて,グルココルチコイド受容体の個体間差異を検討した。本年度はこのデータをもとに難治性アトピー性皮膚炎症例において同様の検討を行い正常成人との比較検討を行なった。グルココルチコイド受容体の検討についてはRT-PCRを用いた半定量法を用いた。1.難治性アトピー性皮膚炎症例より末梢血を採取し末梢血単核球を分離 2.末梢血単核球よりtRNAを抽出しmRNAを分離 3.半定量的RT-PCR法を用い、グルココルチコイド受容体mRNAの発現を検討 4.グルココルチコイド受容体βなどのアイソフォームについても発現を検討 の1から4を行った。その結果、1.正常人においても個体間に受容体mRNA発現量に差異があり、アイソフォームも含め質的差異がある。2.患者群のグルココルチコイド受容体mRNA発現も同様に量あるいは質的差異があるが、正常人コントロールと比較してあるいは重症度と関連づけた一定の傾向は得られなかった。今後はこの結果を元にアトピー性皮膚炎患者のグルココルチコイド受容体発現の量、質的差異をもたらすファクターについてさらに検討を重ねる予定である。なお、本研究は十分なインフォームドコンセントを行い、同意の得られた患者のみを対象として施行した。
